A DISABLED
CITIZEN DOG LICENSE APPLICATION must be completed to be eligible
for the special DISABLED CITIZEN DOG LICENSE FEE. |

Submit
2 copies of this application and current dog license tag. A copy
will be given to you after processing. Please keep that copy,
it may be required for future actions. |

Disabled
citizen is defined by the Americans with Disabilities Act 42 United
States Code § 12102 and Arizona Revised Statutes § 41-1492 (5)
as:
Disability means, with respect to an individual, any of
the following:
(a) A physical or mental impairment that substantially limits
one or more of the major life activities of the individual.
(b) A record of such an impairment.
(c) Being regarded as having such an impairment. |

submit two copies of this application,
and your current dog license tag or a rabies vaccination certificate
signed by a licensed veterinarian. A copy of your application
will be given to you after processing. Please keep that copy,
it may be required for future actions. |

A $5.00
late fee will be charged when applicable, according to City of
Tucson Code § 4-87. |
